Terms & Limitations
*By enrolling in one of our membership plans, member agrees to pay the activation fee at sign up and continue making payments for 1 year from the date of sign up.
*Payments will be automatically charged to your card on file. If we are unable to process the monthly payment, you will have 30 days to update your payment information and bring your account current. If payment is not received before the 30-day grace period ends, there will be a $69 reactivation fee, and all past due payments are due at the time of reinstatement.
After 6 months of non-payment, new membership with full activation fee is available 1 year after the 6 month non-payment period.
*Effective date of this membership plan is on the date agreement is signed.
*Membership will automatically renew 365 days after the agreement date, unless cancelled in writing prior to renewal.
*All membership covered services must be used within 364 days of enrollment and cannot be carried over or used past that time period.
*Payment for any/ all services not covered by your membership is due in full at the time of service to receive 20% discount.
*Membership benefits are only for use at Dahle Family Dentistry
*This plan is non-transferable, has no cash value, and may not be redeemed for cash.
*Membership plan is refundable up to 30 days after start date if no services have been used.
*You may cancel your membership at any time in writing; if membership is cancelled before fulfilling the 1-year membership period, a waiting period of 18 months will be required before renewing membership. No refunds for used portion of membership.
*Eligible for dental services only. Products are not included.
*Rates are subject to change annually.
*Orthodontics not included. Appliances not included.
